What does membership in this practice include?

Each patient receives a comprehensive annual physical exam.  Included in this exam is a preventive health assessment reviewing all necessary periodic health care screenings, an in-depth discussion about nutrition and exercise, as well as recommendations for routine vaccinations.  An annual flu shot is included with membership.  All other routine vaccinations are offered at cost.  All patients will receive an unlimited number of follow-up and acute office visits, telephone consultations, and e-mail communications.  In-office EKG's, urinalysis and other office-based laboratory studies are also included.

How much does membership cost?

There is a monthly fee charged at the end of each month.  Payments can be made either through automatic deduction from your checking account of from your Health Service Account debit card.  Please contact our office for more details.

Is the cost covered by insurance?

While Dr. Rosenberg does not directly participate in any insurance plan including Medicare, patients may be able to use "pre-tax dollars" from a Health Savings Account or Flexible Savings Acount to pay for the monthly fee.  In addition, our office can provide you with a statement for your annual physical exam or any other office visit that you receive.  These statements may be submitted to your insurance company for reimbursement, depending on your plan.

What if I need to see a specialist?

Dr. Rosenberg refers to an extensive network of highly qualified medical specialists and allied health professionals.  Patients can use their regular medical insurance for these services as well as any laboratory and radiology services as these are typically covered by most insurance plans. 

How do I reach Dr. Rosenberg?

During regular office hours, patients may call Dr. Rosenberg's office and request a return call.  Patients may also e-mail Dr. Rosenberg directly with non-urgent matters.  Dr. Rosenberg can be reached after hours by calling our main phone number.  The answering service will connect you with Dr. Rosenberg or the on-call physician.  All of these services are included with the monthly membership fee.
